Chronic Disease Management

These are provided for those patients with ongoing medical conditions. They are a plan created by your doctor to monitor your ongoing treatment and ensuring that those involved in you medical treatment are adequately informed. They set individual goals so that you are aware of what is required of you to manage your condition.

Home Health Assessments

We currently have two nurses that are employed by the VMC to call at your home to prepare a Health Assessment. If you are over 75 you are eligible to have a Health Assessment carried out. This is a government strategy to ensure that you receive as much help and guidance to enable you to remain in your own home. This service is bulk billed, your doctor will seek your permission to have our nurse call and complete the assessment, should you require any information please do not hesitate to speak with your doctor.

Home Medication Reviews

A home medication review can be arranged by your doctor for those patients taking more than five medicines or for those taking Warfarin. Our doctor can arrange a home visit by your pharmacist who will discuss your medications, side effects and any questions you may have regarding any of your medicines. This includes over the counter and herbal medicines.

Immunisations

A full range of child immunisations are available and should be discussed with your doctor. We conduct annual flu clinics in the Autumn.

Travel Information

For those who are planning to travel overseas, it is advisable to check with your doctor, who is able to provide you with information regarding overseas travel. It is also advisable to make an appointment well before your planned departure date.

Test Results

A registered nurse is available between 1.30pm and 3.30pm each day Monday to Friday for patients to call in on a direct line to check the results of your recent pathology or x-ray test. The nurse is only able to access these results once your doctor has read them. The doctor will inform the nurse if they would like you to make an appointment, either urgently or non urgently to review the results. If the doctor has labelled the result “No Action” then the doctor will be happy to discuss the result with you at your next appointment. As it is not appropriate for our nursing staff to analyse your results over the phone, should you require further information it is expected that you will make an appointment with your doctor. Women’s Health

We conduct women’s health clinic fortnightly under the auspices of the Southern Fleurieu Health Services.
